Q: what is acne?
ans: medical term for acne is acne vulgaris, commonly known as 'pimples'.

Q: why does it happen?

ans: this condition is usually common in adolescence n young adulthood.reasons r many:


  • androgens i.e. spurt of hormone during puberty,leading to development of sebaceous glands.
  • heredity i.e. genetic which means excessive sebaceous gland activity is probably due to variation in end organ response to androgens.
  • folloicular keratinization i.e.due to abnormality of keratinazation of hair follicle.
  • propionibacterium acnes i.e. kind of bacteria that grows in subjects with acne.
  • occasionally acne may persist in middle age or rarely begin during middle age, most in men than women. if it is a women then endocrinologic cause is involved.oil base make up/sweating may aggravate acne. premenstrual flare is common.


Q: what are treatment options?

ans:as follows:


  • 2-4 times/day washing of face with facewash n lukeworm water.
  • topical therapy: for grade 1 acne- adapalene or retinoic acid (0.025-0.05%), for grade 2 acne- add clindamycin (1%) or erythromycin (2%) to check bacterial count. depending upon severity add benzoyl peroxide 2.5-5%, sulfur 2%, resorcinol 1%). there is no side effect except irritation. retinoic acid can cause photosensitivity hence must be applied at night n also at initial worsening of lesions. wash after sometime. as the tolerence increases keep it for longer duration.
  • systemic therapy: for grade 1 & 2- vit A 1L units per day orally. add anti bacterial tab roxithromycin 150mg BD, if required.(for grade 2-3-4).
  • for grade 3 &4- all above plus n in women add anti androgens i. e.  tab cyproterone 2mg with cyclical estrogen, though may lead to break through bleeding n vaginal candidiasis.
  • for grade 3&4- all above plus n in men oral retinoids-isotretinoin 20-40mg OD for 4-8weeks. oral retinoids are contraindicated in women, n must be give with written consent.
  • Other modalities: for grade 4 acne-application of dry ice (co2 laser), exposure to UV light to induce peeling (Laser), aspiration of acne cysts, injection with triamcilolone acetonide suspension.
  • scars that follow are treated with Dermabrasion, exicision, collagen injection.
  • then other cosmetic procedures to have glowing skin.
  • use of cleanser (Dry skin-aloe vera cleanser, oily skin-mint/neem cleanser, normal skin-turmeric cleanser, mint face wash for sensitive skin) n toner (dry skin-skin fresh lotion, oily skin-papaya enzyme lotion) n sunscreen (oily skin-aloe vera sunscreen gel spf20 or lakme sunexpert spf24, normal to dry skin-cucumber sunscreen lotion spf30) on regular basis.
  • peeling treatment for glowing n wrinkle free skin every 15 days for rest of life.
  • Botox injections every year to have wrinkle free skin n to maintain youth.
Grades of acne:
Grade1: whitehead i.e. closed comedones i.e. 1-2mm in diameter with central whitish dot represnting a follicular opening.-earliest lesions of acne. (skin coloured papules). Blackhead i.e. open comedones i.e.conical papule with a central dilated follicular pore.
Grade2: 2-4mm in daimeter-follicular papule few topped with pustules.
Grade3: more than 5mm, papulonodular acne.
Grade4: nodulocystic acne, usually progress from painful nodule to painless cystic swelling that rupture n heal with scars. also called acne conglobata.

Q: How can I get rid of blackheads?

A: Products containing salicylic acid or glycolic acid may help to treat blackheads, but if they are persistent, it would be best to see a dermatologist for consultation. A dermatologist can give you prescription creams containing tretinoin or adapalene which are used to treat the condition. These are derivatives of Vitamin A and are the treatment of choice for comedonal acne, or whiteheads and blackheads. They work by increasing skin cell turnover promoting the extrusion of the plugged material in the follicle, and prevent the formation of new comedones.
